Kaleidoscope Health welcomes referrals from therapists, primary care providers, hospitals, and other healthcare professionals for adults ages 18 and older.
Matthew Appling, DNP, APRN, PMHNP-BC, provides psychiatric evaluations and ongoing medication management through secure telehealth appointments.
Who We Can Help
We welcome referrals for adults seeking psychiatric evaluation or medication management for concerns including:
• Anxiety and panic
• Depression
• ADHD and attention concerns
• Bipolar and other mood disorders
• Trauma-related symptoms
• Sleep concerns
• Other psychiatric concerns requiring evaluation
Patients must be physically located in Florida or Georgia during their telehealth appointments. Our team reviews each referral to determine whether our services are an appropriate fit.
How to Refer
1. Contact Our Office
Call 770-568-6061 to discuss the referral process and arrange a secure way to provide any patient records. Patients may also contact us directly and mention your practice.
2. Complete Intake & Verify Insurance
Our team helps the patient complete the required intake forms and verifies insurance benefits before scheduling.
3. Schedule the Appointment
Once intake and insurance review are complete, our team helps the patient schedule a secure telehealth appointment with Dr. Appling.
Working with referral partners
Coordinated Care for Your Patients
We value collaboration with referring professionals. With the patient’s authorization, when required, we can coordinate with their therapist or other healthcare providers to support continuity of care.
Our services focus on psychiatric evaluation and medication management. Patients who already have a therapist can continue that relationship while receiving psychiatric care through Kaleidoscope Health.
