Getting started with Kaleidoscope Health is simple and secure. We provide personalized online psychiatric evaluations and medication management for adults 18 and older in Florida and Georgia.

HOW IT WORKS

1. Contact Our Office

Reach out to Kaleidoscope Health to request new patient registration. Our team will help you get started.

2. Complete Your Secure Intake Forms

You will receive an email invitation through our system to complete your medical and psychiatric history, insurance information, and required consent forms online.

3. Insurance Verification & Appointment Scheduling

Once your completed intake forms have been received, our office will review your information, verify your insurance benefits when applicable, and contact you to schedule your initial appointment.

4. Meet Your Provider

Connect securely with your psychiatric provider from the comfort of your home through our convenient telehealth services.

YOUR FIRST APPOINTMENT

Your initial tele-health appointment is approximately 60 minutes. Your provider will conduct a comprehensive psychiatric evaluation, discuss your concerns, and develop an individualized treatment plan tailored to your needs.

FOLLOW-UP APPOINTMENTS

Follow-up visits typically last 30 minutes. Your provider will review your progress, discuss your treatment response, and make adjustments when clinically appropriate. We will utilize rating scales to monitor your progress, assess the effectiveness of your medication and therapies, and make any necessary adjustments to your treatment plan as needed.

INSURANCE & PAYMENT

We accept many major insurance plans and Medicare. Coverage may vary by plan, so please verify your benefits before your appointment.

New Patient Registration

Getting started with Kaleidoscope Health is simple, secure, and completely online.

We provide psychiatric evaluations and medication management through telehealth for patients in Florida and Georgia.

Ready to get started? Contact our office to request your secure new patient intake forms. Once your completed forms have been received and reviewed, our team will contact you to schedule your initial appointment.

CANCELLATION POLICY

A $125 fee may apply to missed appointments or late cancellations.

Please contact our office at least 4 hours before your scheduled appointment if you need to reschedule.
