Getting started with Kaleidoscope Health is straightforward. Our tele-health appointments provide personalized psychiatric care from the comfort and privacy of your home, ensuring you receive the behavioral health services you need.

HOW IT WORKS
1. Request Your Appointment
Choose a convenient appointment time.
2. Complete Your New Patient Intake Forms
Fill out and submit the required new patient intake forms.
3. Verify Your Insurance
Our office will review your insurance information.
4. Meet Your Provider
Connect securely for your tele-health appointment.
YOUR FIRST APPOINTMENT
Your initial tele-health appointment is approximately 60 minutes. Your provider will conduct a comprehensive psychiatric evaluation, discuss your concerns, and develop an individualized treatment plan tailored to your needs.
FOLLOW-UP APPOINTMENTS
Follow-up visits typically last 30 minutes. Your provider will review your progress, discuss your treatment response, and make adjustments when clinically appropriate. We will utilize rating scales to monitor your progress, assess the effectiveness of your medication and therapies, and make any necessary adjustments to your treatment plan as needed.
INSURANCE & PAYMENT
We accept many major insurance plans and Medicare. Coverage may vary by plan, so please verify your benefits before your appointment.
Complete your intake forms to get started with personalized mental health care, sign/date and submit to om@kaleidoscopementalhealth.com with a copy of your insurance card (front/back) and copy of your ID.
Once approved you will be emailed a confirmation. All forms should be completed/signed prior to your first appointment to ensure adequate time for consultation.

CANCELLATION POLICY
A $125 fee may apply to missed appointments or late cancellations.
Please contact our office at least 4 hours before your scheduled appointment if you need to reschedule.

Mental Health Appointments at Kaleidoscope Health
Please complete all required new patient intake forms before your first appointment for our behavioral health services.
