NEW PATIENTS
Your initial tele-health appointment will take approximately 1 hour. We will do a full evaluation and diagnosis, and will individualize a treatment plan specific to your needs; including any medications and/or recommended therapies.
FOLLOW-UP APPOINTMENTS
This tele-health visit type will take approximately 30 minutes. We will use rating scales to monitor your condition, discuss how the medication/therapies are working, and make any necessary modifications to your treatment plan if appropriate.
We accept most major health insurance policies, as well as Medicare. If your insurer does not cover behavioral health services we can provide you with a fee schedule for services.

Click on the new patient intake forms button and simply print all forms, complete, sign/date and submit to om@kaleidoscopementalhealth.com with a copy of your insurance card (front/back) and copy of your ID.
Once approved you will be emailed a confirmation. All forms should be completed/signed prior to your first appointment to ensure adequate time for consultation.
